To register with the Practice patients need to come into the surgery and fill in a registration form. Please bring identification with you. The patient will be offered a health check which includes brief medical history, BP check, urine test and height and weight
New patients on regular medication will need to be seen by a doctor before being issued with a repeat prescription.
Please be aware that as per PCT policy, patients leaving the country for more than 3 months, should register as a temporary patient where they are staying, and medication should be requested from the local practice.
Medication Review dates are given for the benefit of the patient. Please ensure that when your review date is due, you make an appointment with the Nurse for blood tests if required (if unsure please ask at reception) and then with a Doctor. This enables the Doctor to ensure that your prescribed medication is still appropriate for your medical needs.
Summary Care Records
Our Practice enables patients to have an electronic Summary Care Record. This is an NHS initiative which improves the way your health information is stored and managed. Your Summary Care record will be available anywhere in England but access will be via strict security measures.
At first only allergies, bad reactions to medication and prescriptions will be included.
Having a Summary Care Record will help ensure the right people have the right information at the right time. Please ask for a leaflet which will explain about how your health information would be stored and managed and the options you have as a patient, or ask the receptionist who can explain about Summary Care Records to you.
If you are happy to have a Summary Care Record you do not need to do anything and your record will be uploaded as part of the registration process. If you do not want this, you will need to complete an opt out form, please ask at the reception.
